Congratulations on the new addition to your family! If you are current client (been seen at our facility within the last 3 years) and have added a new pet to your family, please fill out a new patient request form so our Client Liaison may contact you to set up the first exam.
We are excited to meet your new fur baby and to provide the needed medical care required to keep your new addition happy and healthy!
In order for our hospital to provide the best care possible, we do require all of your pet(s) previous medical records prior to scheduling an appointment. This can be done via dropping off the physical record, faxing (860-668-4785), emailing (SVH@suffieldvet.com), or mailing (577 East St. South, Suffield, CT 06078).
